About Your Visit
Before Your Visit
- • Schedule your appointment by calling (949) 650-4362 or request an appointment at appointment@CompleteSpine.com
- • Bring all relevant X-rays, MRIs and test results to the office with you.
- • Bring any written referral from your primary care physician or specialist who referred you to our office.
- • Bring your a picture ID and insurance card(s).
- • Be on time to your appointment to assure that you receive adequate time with the doctor. Please give yourself adequate time to fill out new patient forms.
- • If you need to cancel, please call at least 1-2 business days in advance.
During Your Visit
- • Bring any pertinent health issues you may have to your doctor's attention, even if you don't think it may be related to the present reason for your visit
- • Make sure your doctor knows about any recent MRI's, Xrays or procedures (such as epidurals or spinal surgery) that have been done recently.
- • We are here to help you. Please ask any questions or concerns you may have about your health or insurance benefits.
After Your Visit
- • If your symptoms get worse, or if you have problems with your treatment, call us at (949) 650-4362 and ask to speak to your treating doctor.
- • If your doctor said that you need certain tests, make the appropriate arrangements at the facility to get them done and have them fax a copy of the results to our office at (949) 650-4366.
- • Call us to make a follow-up appointment after you have had any outside test performed (MRIs, Xrays, lab tests etc.)
- • If your doctor has referred you to another specialist, make an appointment and please take our business card and have them fax us at (949) 650-4366 their findings so we can have for our records.
- • If you have a test done, don't assume that no news is good news. Ask about the results.
- • If you are waiting for an authorization for further treatment we will call you within a week. If you do not hear from us during this time please call and we will look into this matter promptly.
